Registered Nurse- (Home Base Primary Care)

The RN will provide care coordination and direct patient care for veterans residing in the HBPC program catchment area. The HBPC RN's role includes initial and ongoing assessment/reassessment, monitoring and care coordination during the veteran's entire enrollment period to meet the patient's healthcare needs utilizing the principles of case management. The Home Based Primary Care RN provide for care planning and assessment, care coordination and direct patient care in veterans' or caregivers' private residences as well as Medical Foster Homes and assisted living facilities in collaboration with the HBPC multidisciplinary team. The HBPC RN ensures smooth transition of care to and from home care through diligent and collaborative efforts with multidisciplinary teams within and outside the facility. This would also include co-management of care with outside community agencies, particularly with dual care providers. She/he communicates and coordinates care within the facility and community healthcare agencies including state and federal regulatory agencies as it relates to maintenance of safety and compliance with nursing standards of care and practice. The function of the Registered Nurse is to provide, direct and supervise the delivery of quality nursing care to a culturally diverse veteran population. The Registered Nurse possesses a broad-based clinical knowledge and skills to effectively apply all aspects of the nursing process within a collaborative, interdisciplinary practice setting. The Registered Nurse effectively implements activities towards health promotion and prevention, management of acute and chronic illness states, assisting the veterans in attainment of optimal levels of functioning through rehabilitation and provision of supportive measures for the dying patient and their significant others. The Registered Nurse is directly responsible to the designated Clinical Nurse Manager & Director of HBPC. Home based care nurses administer medications, monitor vital signs, lab draws/INR, Foley catheter insertion/changes, dressing changes, Blood glucose monitoring, Medi-planner assistance and educate patients and their companion on health care issues that are related the patient's conditions. They are the case manager between the facility and the veteran. Again, the nurse is responsible to report all finding to the interdisciplinary team provider therefore CPRS/ computer documentation is very important and required. Work Schedule:

Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Experience:
Minimum of recent 2 years nursing experience in Long Term and Extended Care. Home Base Primary Care Experience preferred but not required. Grade Determinations:
The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience.
